ID Card
Q. How do I request a new member identification card and how long before I receive it?
A. We offer several ways for members to order a new identification card.
- Online ID Card requests will soon be available through CHPConnect
- Call Member Services at 383-3311 to make a request for a new ID card. You can leave an automated voice message with your request by pressing option #3 or you may choose to talk directly to the member services' staff.
It will take approximately 7 to 10 days to receive your card by mail to your address of record (see changing your address).
Address Changes
Q. How do I change my address with CHP?
A. You must contact your employer to have an address correction sent to the plan (feel free to download a Member Status Change Form and submit to your employer for processing with CHP). CHP can not accept forms that have not been processed through your employer.
Privacy of my Information
Q. I am concerned about information privacy and how CHP deals with this as an issue?
A. CHP considers the confidentiality of member information a high priority. Policies and procedures are firmly in place that restrict access to medical records, claims, and other personally identifiable data. These are reviewed at least annually.
