As a Capital Health Plan member it’s important to know about the resources available to you. We strive to make it easy for you to get the care, tests and treatments you need. We continually look for new ways to improve the quality and accessibility of our services. Here are important resources to help you get the care you need when you need it:

Urgent Care Center

CHP’s Urgent Care Center is available when you’re sick and can’t see your primary care physician. Many sicknesses and injuries may cause you to seek immediate help; however, not every case is serious enough for a trip to the Emergency Room. The CHP Urgent Care Center is quick, costs less than the ER, and is staffed by physicians, physician assistants, nurse practitioners and nurses.

Types of medical conditions appropriate for the Urgent Care Center include:

• Upper respiratory infections such as colds, ear infections, bronchitis and sore throats.
• Minor symptoms acute cases, which include fever, nausea, vomiting and urinary tract infections
• Minor trauma including ankle sprains and cuts

When you need immediate care, CHP’s Urgent Care is available by appointment, Monday through Friday from 11am to 10pm, and weekends from 9am to 8pm. Our Urgent Care staff works hard to see you as soon as possible, but we do ask that you call ahead before coming in.

If you want to avoid long wait times, high copayments and do not have a debilitating or life-threatening injury, consider calling CHP’s Urgent Care Center. You can call the Urgent Care Center directly at 383-3382.

CHP Health Coaching

When you’re diagnosed with a medical condition or are facing a health care decision, you may need to talk with a health care professional after your physician’s office is closed. CHP Health Coaching is here to help. CHP Health Coaching is a 24-hour health information and decision support line available to members at no additional cost.

The Health Coaches who staff the line are specially trained health professionals (such as nurses, dieticians, and respiratory therapists). They are available to help you make tough medical decisions or to counsel you on important changes to your health. And, since the coaches are available anytime day or night, they’re ready to give you advice when you need it most.

(If you have a truly life-threatening condition, go to the Emergency Department.)
