CHPConnect: Your Online Health Record
Capital Health Plan offers our members the opportunity to access their CHP medical history, change their primary care physicians, and much more through our CHPConnect.
CHPConnect
With a CHPConnect account, you can review:
- A personal history of your doctor visits and procedures
- Current medications
- Prescripton drug information
- Claims
- Referrals
- Benefits - including your copays
- Diagnoses
- Children's immunizations and visit dates
- Healthwise reference library
- Current health status snapshot
- **COMING SOON** Lab results
You can also request ID cards or make changes to your Primary Care Physician and demographic information.
How do I connect to CHPConnect?
- Log in to CHPConnect.
- On the log-in page, click on the "Member" button on the right.
- Fill in the registration form and choose a password (which must be at least 6 characters - upper and lower case plus a number).
- Capital Health Plan will mail you an access code within 24 hours. If you need access sooner, please call Member Services at 850-383-3311.
- Use your User ID, password and access code to log in.
