Member Services Call Center Monday - Friday, 7am to 7pm* (beginning January 1, 2012)

The Capital Health Plan Health Center at Centerville Place is home to Capital Health Plan’s Urgent Care Center, open seven days a week by appointment. The Centerville facility also offers physicians’ offices, x-ray and lab services, Capital Health Plan’s Colon Screening Program, and an outstanding eye care center. All of these services are available exclusively to Capital Health Plan members!

The Governor’s Square health center offers physicians’ offices, x-ray and lab services, digital mammography, DEXA Scanning, and an outstanding eye care center. This facility is also home to Capital Health Plan’s Center for Chronic Care. All of these services are available exclusively to Capital Health Plan members!
