With a CHPConnect Account you can:

  • Check details of your benefits
  • Check the status of medical claims
  • Check the status of authorizations for specialty care
  • View your CHP medication history
  • Access Healthwise© and get credible medical information
  • Participate in special health improvement programs
  • Choose a primary care physician
  • Replace your member ID card
  • Update your address

Register for CHP Connect

Registering for CHPConnect is easy. You'll use three simple screens. Here are tips.

  1. Have your Member ID number ready.
  2. On the first screen, click the Members picture.
  3. Next, fill in the blanks. Choose Capital Health Plan for the last one.
  4. On the final screen, choose your own password and security clues. Click ? for directions.
  5. Download the Membership Agreement form. (pdf 81.96 kB) In order to activate your account, we must receive a signed copy of the form. Print, sign and return it to
