6 Steps to Get Connected:
So get your member ID number, go to your computer, follow these fast steps, and connect.
- Go to www.capitalhealth.com and click CHPConnect. On the next screen, click MEMBER LOGIN (bottom right).
- On the next screen (labeled "HealthTrio"), click the bottom right icon MEMBERS. Fill in the registration form that appears and click NEXT.
- First, enter your member ID number only (not preceded by CHP); enter your birth date as MM/DD/YYYY; then create a password (six characters, mixed case, plus a number).
- Print User ID and Member Agreement.
- Read, sign, and mail the Member Agreement in this HealthLine. CHP must receive the form with your original signature to activate your account.
- Within 3-5 business days, CHP will send you a personal Access Code.
