Registering is easy. Go to www.capitalhealth.com, take the tour, and sign up. Then submit the postage-paid form we've included in this HealthLine.

6 Steps to Get Connected:

So get your member ID number, go to your computer, follow these fast steps, and connect.

  1. Go to www.capitalhealth.com and click CHPConnect. On the next screen, click MEMBER LOGIN (bottom right).
  2. On the next screen (labeled "HealthTrio"), click the bottom right icon MEMBERS. Fill in the registration form that appears and click NEXT.
  3. First, enter your member ID number only (not preceded by CHP); enter your birth date as MM/DD/YYYY; then create a password (six characters, mixed case, plus a number).
  4. Print User ID and Member Agreement.
  5. Read, sign, and mail the Member Agreement in this HealthLine. CHP must receive the form with your original signature to activate your account.
  6. Within 3-5 business days, CHP will send you a personal Access Code.
