Membership Updates
Away from Home Care Participants. If you or your child is enrolled in Away from Home Care, remember that you must reapply every year for the program. Renewal is not automatic. We want to keep your benefits active, so be sure to call Member Services at 383-3311. Not sure of your renewal date? Just ask us.
Please Update Information During Open Enrollment. If this is your employer's open enrollment period, it is time to be sure your CHP information is correct. Lives change marriage, a new baby, a new address, a new phone number and we need updated records so that your membership continues without interruption. Tell your employer or go online to CHPConnect.
