CHPConnect is exclusive in our area: a secure, online personal health record available to all members. Never again forget what shots your child has had or the date of your last mammogram! Safe and password-protected, CHPConnect is an invaluable, at-home, 24-hour tool for you and your family's health. You can access information about physician visits, referrals, scheduled procedures, prescriptions, benefits, and copayments.
Besides these records, CHPConnect also gives you HealthwiseHealthwise®#174;, an easy to understand health information source. Written and reviewed by doctors, HealthwiseHealthwise®#174; includes health topics, symptoms, medications, tests, and wellness. It's a powerful tool for your health.

CHP's Urgent Care Center is not just for nights and weekends. This unique, patient-friendly centerstaffed by nurses and physiciansprovides urgent care weekdays, noon-11 p.m., and weekends, 9 a.m.- 8 p.m., at our Centerville Place Health Center.
For severe colds, urinary infections, minor injuries, and other urgent but nonemergency conditions, the Urgent Care Center is a tremendous resource. Always call 383-3382 before coming to a time arrange for you to be seen and avoid waiting. (For a true emergency such as a heart attack or major wound, go immediately to the hospital Emergency Room. Your CHP membership covers necessary ER care with a copay.)
Away from Home Care is portable peace of mind. This program that provides health care coverage for members living away from home for at least 90 consecutive days. It is a great boon to college students, members on long-distance work assignments, and eligible children living with a custodial parent in another area (members must be non-Medicare).
Away from Home Care allows you to apply for a guest membership in a Blue Cross and Blue Shield HMO, available in other 200 cities and every state. Guest members have a primary care physician and pay only required copays, with no claim forms to complete.
Enrollment is easy. Call Member Services at 383-3311. We'll determine if there is a participating HMO at your destination and help you complete the application.
A Nationwide Walgreens Affiliation means that prescriptions for out-of-area urgent and emergency care, and for members enrolled in Away from Home Care, may be filled at any Walgreens pharmacy in the nation4,800 stores. Simply present your CHP membership card, and your CHP prescription benefit and copayments apply.
