Follow These Easy Steps & Connect to CHPConnect

You can do it! CHPConnect, an online personal health record, is up and running at www.capitalhealth.com. Ask members who have already signed on: 24-hour CHPConnect answers questions, saves time, and simply gives you more control of your health. You can review your doctor visits, diagnoses, referrals, medications, children's immunizations, benefits and copays, and a great health library.

We've made CHPConnect highly secure for you, according to your plan's privacy principles and the law. Registration takes only a few simple steps. You sign in, select a password, read and sign an agreement, and receive an access code. For your convenience, the agreement is inserted in this issue of HealthLine.

So get your member ID number, go to your computer, follow these fast steps, and connect.

  1. Go to www.capitalhealth.com and click CHPConnect. On the next screen, click MEMBER LOGIN (bottom right).
  2. On the next screen (labeled "HealthTrio"), click the bottom right icon MEMBERS. Fill in the registration form that appears and click NEXT.
  3. Create a password (six characters, mixed case, plus a number).
  4. Read, sign, and mail the Member Agreement in this HealthLine (the form is also downloadable online). CHP must receive the form with your original signature to activate your account.
  5. Within 3-5 business days, CHP will send you a personal Access Code.
