Performance Drug List for 2008
If your prescription benefit plan has four cost-sharing tiers, you will pay the Tier 4 copay/coinsurance for Copaxone and Rebif, which are both self-injectable drugs on the 2008 Performance Drug List. During the year other self-injectable drugs may be added to the Tier 4 copay/coinsurance.
