	Choose a primary care physician (PCP) for yourself or a family member. You can search for a physician on our here.
	Transfer your medical records if you are choosing a new doctor. Complete the Authorization to Disclose Protected Health Information Form [PDF] and give it to the office that now has your medical records.
	Register for CHPConnect. Manage your health care online (create a personal health history, track doctor visits, tests, referrals, diagnoses, and more).
	Learn how to get care as a CHP member. Read Understanding Capital Health Plan [PDF], Urgent Care Center [PDF], Away From Home Care [PDF], and Your Other CHP Benefits [PDF].
	Find the prescription drugs that you need. Check the Commercial Member Preferred Medication List.
	Find the CHP centers. See the location map.
	Find where to get lab services. Read Tips for Better Laboratory Services [PDF].
	Learn all about your benefits. Consult your Capital Health Plan Member Handbook and endorsements.
	Review the Member's Rights & Responsibilities and Privacy Notices.
	Complete and mail the Coordination of Benefits Questionnaire [PDF] within the first 30 days of coverage.
