Health Calendar
With this calendar, you can keep tracks of appointments and tests at a glance, as well as write notes. The add feature lets you insert reminders about everything from exercise to immunizations - and even type in questions to ask your doctor. you can also link to CHP's Preventive guidelines and incorporate recommended screenings for your age. this sample shows a month, but you can display the calendar as a week, day, or list.
Medical History
The Medical History shows, in chronological order, both medical procedures and immunizations you've had, by whom, and where. CHP has inserted data beginning from January 2003. The history also lets you add important notes, for example, symptoms or an allergic reaction. more help for your memory!
Medication Profile
The prescritions you fill will appear in the Medication Profile, showing the last date filled, dosage, and remaining refills. With a click on the medication, you also get detailed information about the drug. Notice the interactive buttons above the list. They let you make necessary changes, list vitamins and over-the-counter drugs you take, and even add a note - perhaps about a reaction.
Look also at the top three tabs. The profile is divided into drugs you are currently taking, a history of your prescriptions, and your known allergies. You see why instant access to these facts is important for you, your doctor, and specialists.
My Plan for Health
Visits to your primary care physician and specialists yield your personal health issues - perhaps cholesterol, smoking or a needed preventive screening. Here you can chart a plan for these issues, including notes and reminder dates. Also, you can click on the symbol, to learn more about each condition or procedure. Personalize your plan even more by adding your own goals, such as daily walking. With you own Plan for Health, you take charge of your future.
