Benefits and Eligibility

Change PCP
Finding and selecting a primary doctor for you or your children has never been easier, when you register for CHPConnect. A simple form lets you search for a physician, entering your personal preferences. You'll quickly find out which PCPs in our large network are taking new patients. Once you've made your choice, click on accept.
ID Card Request
This screen lets you send CHP Member Services a request for an ID card - for yourself or anyone covered under your plan. Each person's name and member identification number is listed, with a checkbox. As another convenience, you can print a temporary ID card to use until the card arrives in the mail. And if you have questions about ID cards, the Frequently Asked Questions give you quick answers or where to turn for help.
Member Information
Whether you've changed your address or gotten married, the Member Information page lets you quickly update your records directly, accurately, at any time of day or night. Keeping your data current is vital to receive CHP services, and now you can do it yourself. Remember, though, that you must still notify your employer of any status changes.

Providers Search
When you're looking for a doctor, you have personal preferences and you want as many choices as possible. This search page connects to our extensive, up-to-date database of participating physicians, primary care doctors and other specialists. Notice that you may fill in several factors to narrow your search, such as gender and location, or leave blanks for a wider search. Once you've filled in the form, hit Search, and CHP
Referrals & Authorizations
When your doctor refers you to a specialist, or authorizes a hospital stay, this part of CHPConnect assures you that everything is in order. The listing shows whether the request is approved, the referral number, and the time period. Click on the referral number for even more details, such as the number of authorized visits and a description of services.
