Healthy Lifestyle Rebate Program
The health and well-being of your employees are important to us! The Healthy Lifestyle Rebate Program offers premium rebates for Small Group accounts whose CHP covered employees meet the "healthy lifestyle" criteria. A small group is defined as those having no more than 50 employees. The program offers premium rebates of:
- $50 for employees who quit smoking
- $50 for employees who reach a healthy Body Mass Index (BMI)
- $100 for employees who accomplish both of the above goals
- $25 for non-smoking employees who maintain a healthy Body Mass Index (BMI)
The rebates are paid directly to the employer for the group account. The enrollment period for the 2007 calendar year program is from January 1, - May 31, 2007.
