CHPConnect

Registering for CHPConnect is easy. You'll use three simple screens. Here are tips.

  1. Have your Member ID number ready.
  2. On the first screen, click the Members picture.
  3. Next, fill in the blanks. Choose Capital Health Plan for the last one.
  4. On the final screen, choose your own password and security clues. Click ? for directions.
  5. Download the Membership Agreement form. In order to activate your account, we must receive a signed copy of the form. Print, sign and return it to
