Notice: Capital Health Plan Recorded Phone Calls

Call monitoring is an effective method for improving the level of service provided to our customers, our patients and our members.

Part of Capital Health Plan’s overall strategy is to provide top-level customer service throughout the organization, including telephone etiquette and accurate information. Incoming and outgoing calls are recorded in departments in which our staff routinely communicates with you. When our staff places an outbound call to you, they will indicate that the call is recorded, when applicable. You may also hear beeping during your call, letting you know your conversation is being recorded.

Question:

What is a Primary Care Physician and how do I choose one for myself?

Answer:

One of the most important health care decisions you will ever make is selecting your primary care physician. This is your health care partner and central coordinator for everything pertaining to your good health. Primary care physicians include family physicians, internal medicine physicians and pediatricians.

The process for choosing your primary care physician is simple.

1) First, find a physician accepting new patients:

  • Access the CHP Physician Search Tool. Visit www.capitalhealth.com and on the homepage, under the phrase “Learn more about,” click on the word “Doctors”
  • On the Physician Search tool, click the “advanced search” link.
  • Use the drop down box in the category labeled “Accepting Patients” to choose “Yes”.
  • Check the box next to the words “Primary caregivers only” and then click “Go”. (If you want to narrow the search further, enter the zip code, city, medical group affiliation, and/or other search items accordingly.)
  • Review the list of primary care physicians accepting new patients and choose the one that you would like to have as your health care partner.

2) Then, notify your local Member Services Representative of your decision so our records will reflect your choice.

  • Log on to your CHPConnect account and make your selection. This automatically sends a secure message to the Member Services staff; your selection will then be completed that evening. (You can register for a CHPConnect by visiting www.capitalhealth.com and clicking on the link titled
    in the left hand column.)

Question: My address and/or phone number has changed. How do I update them with CHP?

Answer: It is very important that Capital Health Plan has a correct address and phone number on file for you and your family. During the year we send out several important pieces of literature to explain changes to your benefits, to announce changes to our physician and provider network, or to assist you in optimizing your overall health.

How to correctly update your address:

1) Notify your employer. Notify your employer’s HR office of your change in address. Your employer will forward this information to us to update our records.

2) Notify Capital Health Plan. Mail or fax in a notice to Capital Health Plan’s Enrollment department (fax # 850-523-7369).

The notice must be signed by the subscriber of the policy. You may also notify us through a secure message on CHPConnect. Since CHPConnect includes a secure messaging system, information received through this application allows us to accept and complete your request to change your address. You can register for CHPConnect by visiting capitalhealth.com and clicking on the Member Login (CHPConnect) link.

Address changes cannot be taken over the phone by your Member Services Representatives. However, you may request changes to your phone number by contacting CHP Member Services.

Making It Easy to Get the Care You Need

At Capital Health Plan, we strive to make it easy for you to get the care, tests and treatments you need. We continually look for new ways to improve the quality and accessibility of our services. Here are some of the ways that we have worked to make it easy for you to get care:

 

  • Capital Health Plan’s large network now includes over 125 Primary Care Physicians and over 300 Specialists.
  • Most local participating specialists do not require prior authorization for an appointment.
  • Preventive screenings such as mammograms and colon screenings do not require an authorization prior to the services.
  • Two CHP Eye Care Centers provide vision services when you need it.
  • CHP has a network of over 62,000 participating pharmacies nationwide to make it easy and convenient to fill your prescriptions.
  • When you need immediate care, CHP’s Urgent Care is available by appointment, Monday through Friday from 11am to 10pm, and weekends from 9am to 8pm.
  • CHPConnect allows you access to your medical information and test results 24/7.
  • While traveling outside the service area, CHP provides coverage for emergency services through Blue Cross Blue Shield providers nationwide.
  • Our local Member Services Staff is Available to Assist you and answer your questions.
