Capital Health Plan encourages you to stay physically active so you can Live Well! For that reason, we offer reimbursement on your fitness center membership as well as special discounts at local fitness centers (exclusively for our members).
Health & Fitness Reimbursement
Capital Health Plan members can receive up to $150 per calendar year (per household) for membership at a qualified health and fitness center during that year. Check here for information on how the program works and to find the forms you need.
