Health Promotions supports the development of healthy lifestyle habits. Lifestyle change is encouraged through a combination of experiences that promote awareness and motivation. Capital Health Plan has created wellness and disease management programs to encourage members to adopt a healthier way of life and prevent the onset or worsening of an illness or disease. The ultimate goal is to make positive health practices the easiest choice.

Capital Health Plan provides access to recommendations for screening tests, counseling, and immunizations to help prevent illness.

Capital Health Plan offers our members the opportunity to access their Capital Health Plan medical history, change their primary care physicians, and much more through our CHPConnect.

The Health Information Line is a 24-hour/day phone line staffed by health care professionals who are able to assist you with your health related questions. While not a substitute for a visit with your physician, the Health Information Line staff can provide you with tips, tools and resources to help you manage your health.

Capital Health Plan offers wellness programs that are free for members. Members may access classes directly by dialing the phone number listed in the program description. If you need additional information, please call the CHP Health Information Line.

Capital Health Plan encourages you to stay physically active so you can Live Well! For that reason, we offer reimbursement on your fitness center membership as well as special discounts at local fitness centers (exclusively for our members).
