Using CHPConnect, you can review:

  • A personal history of your doctor visits and procedures
  • Current medications
  • Prescripton drug information
  • Claims
  • Referrals
  • Benefits - including your copays
  • Diagnoses
  • Children's immunizations and visit dates
  • Healthwise reference library
  • Current health status snapshot
  • Lab results

You can also request ID cards or make changes to your Primary Care Physician and demographic information.

How do I connect to CHPConnect?

  1. Members: Click Here to log in. (Continue to Step 3).
  2. Employers, Brokers, and Providers: Click Here. On the log-in page, select your role from the options on the right.
  3. Fill in the registration form and choose a password (which must be at least 6 characters - upper and lower case plus a number).
  4. Capital Health Plan will mail you an access code within 24 hours. If you need access sooner, please call Member Services at 850-383-3311.
  5. Use your User ID, password and access code to log in.
