Diabetes Disease Management
The Diabetes Disease Management Program is designed to provide educational tools and reminders, assisting members to make healthy lifestyle choices which support the physician/patient self-management plan. The goal is to improve blood sugar control and prevent major complications such as stroke, heart attack, amputation, kidney failure and blindness.

- Capital Health Plan waives the annual dilated eye exam co-pay for their diabetics when the exam is performed at either one of CHP’s Eye Care centers or at Alpha Eye Clinic.
- Tallahassee Memorial Healthcare Diabetes Center
- The TMH outpatient Diabetes Center is staffed with nurses and dietitians who are certified diabetes educators. CHP members must first contact their primary care office to obtain a referral. TMH will schedule the appointment. If you have been seen at the center in the past and would like to attend again, call the number listed on the TMH website to schedule an appointment.
