Finding resources to assist you in becoming an effective self manager of your chronic illness can sometimes require a little detective work. Big Bend 211, Inc.Creating a Caring and Connected Community

The Community Resource Directory is compiled by 2-1-1 Big Bend, Inc., a nonprofit organization. It is a resource to human services in Florida’s Big Bend Counties, including, Franklin, Gadsden, Jefferson, Leon, Liberty, Madison, Taylor and Wakulla. The organization’s mission is to help individuals and communities by bringing people and services together through programs that include telephone counseling, crisis intervention, and resource information. Referral and training services are also available. You may access Big Bend directory assistance by simply dialing 2-1-1 on your telephone or by visiting Big Bend 211's resource directory online.

Health Crossroads® Web Modules

CHP Health Coaching Shared Decision-Making® programs are available on various medical conditions and tests. Each feature video explains treatment or testing choices and outlines the possible risks and benefits of each choice. Real patients explain what they went through when making tough medical decisions. The web modules are available to view online through CHP Health Coaching or CHPConnect. Videos are also available for home television viewing on DVD or VHS by calling a health coach at 850-383-3400.
