Disease Management
A comprehensive program of disease management, health coaching, decision support, symptom management, and health and wellness education is provided for CHP members who have asthma, diabetes, coronary artery disease, congestive heart failure and chronic lung disease.
These disease management programs are offered to CHP members through an innovative Collaborative Care Program provided by Health Dialog Inc. The Collaborative Care Program is designed to support and reinforce treatment plans of each member's primary care physician.
Capital Health Plan members do not have to enroll; they are automatically enrolled when they are identified as having one of the chronic conditions listed above. Physicians can also refer members to the Collaborative Care Program. Log on to CHPConnect for referral forms and contact information.
