Capital Health Plan Advantage Plus, Preferred Advantage, and Retiree Advantage are HMO plans with a Medicare contract.  Enrollment in Capital Health Plan Advantage Plus, Preferred Advantage and Retiree Advantage depends on contract renewal.  Benefits, formulary, pharmacy network, premium and/or copayments/coinsurance may change on January 1 of each year.  Limitations, copayments, and restrictions may apply.  Please call Capital Health Plan Member Services Department to obtain documents in alternate formats or languages. To ensure that beneficiaries receive appropriate care, Capital Health Plan will follow policies and procedures as directed by CMS (Centers for Medicare and Medicaid Services) in the event of an emergency situation designated by the Department of Health and Human Services.
