2012 Capital Health Plan Advantage Plus (HMO)
Capital Health Plan Advantage Plus - $30 per month
With Capital Health Plan Advantage Plus (HMO) there are no deductibles to pay. Instead, you pay a monthly plan premium and copayments or coinsurance for services you receive.
You can join Capital Heath Plan Advantage Plus if you are entitled to Medicare Part A and enrolled in Medicare Part B. You must continue to pay Medicare Part B premiums. You must be a permanent resident of our service area which includes the Florida counties of Calhoun, Franklin, Gadsden, Jefferson, Leon, Liberty and Wakulla. You cannot join a Capital Health Plan Medicare Advantage (HMO) plan if you have end stage renal disease (ESRD) unless you are a member of our organization and have been since dialysis began.
