Medication Therapy Management Program

Capital Health Plan offers Medication Therapy Management by a clinical pharmacist to eligible members.

The purpose of this program is to promote appropriate, safe, and cost-effective drug use.

Members meeting all three of the following criteria will be invited to join the program:

  1. Have at least three of the following chronic diseases: high blood pressure, heart failure, diabetes, and osteoporosis.
  2. Take at least eight Part D covered medications; and,
  3. Be projected to have a total annual drug cost of at least $3,000 for all covered Part D medications.

Eligible members will be mailed a letter of invitation. If eligible, you will be given the opportunity to meet one-on-one with a clinical pharmacist for a complete medication review. In addition to answering your medication questions, any issues identified at this visit will be communicated with your primary care doctor. At a minimum, you will remain enrolled in the program for the duration of the benefit year and will have the opportunity to meet and/or speak with the clinical pharmacist as needed.
