Capital Health Plan offers employer-sponsored group health coverage for

  1. Large group employers (51+ eligible employees)
  2. Small group employers (2 - 50 eligible employes)
  3. One-life employer groups (Special Open Enrollment in the month of August with an October 1 effective date.)
