Existing Agents: CHP Connect

• Existing agents should go to the CHPConnect broker portal to obtain information about their existing groups.

Quotes will include different information and forms for different group sizes:

• Groups with 51+ eligible employees
  • Group Census
  • Current rates, renewal rates and plan design.
• Groups with 1-50 eligible employees
  • Group census
  • Medical Questionaire
  • Plan design

Processes for:

Submitting New Business

• Required for New Large Group
  • Completed New Group Enrollment Recap
  • Enrollment Applications signed by Authorized Group Representative & Employee
  • First months premium check
  • Completed and signed (by Authorized Group Representative) Employer’s Application for Membership
  • Completed Business Bio Form with Contact Information
  • Must meet underwriting guidelines for Groups with 51+ eligible employees. Contact the Sales Department at 850-523-7333 for more information.
• Required for New Small Groups
  • Completed New Group Enrollment Recap and appropriate tax documentation.
  • Enrollment Applications signed by Authorized Group Representative & Employee
  • First months premium check
  • Completed and signed (by Authorized Group Representative) Employer’s Application for Membership
  • Completed Business Bio Form with Contact Information
  • Must meet underwriting Guidelines for Groups with 2 - 50 eligible employees. Contact Sales Department, 850-523-7333, for more information.

Submitting Renewal Business

• At least 45 days prior to the renewal date, the group will receive a renewal packet regarding rates and benefit plans. Instructions will be included in the packet.
