Capital Health Plan offers employer-sponsored group health plans in Leon, Gadsden, Jefferson and Wakulla Counties in North Florida.
Capital Health Plan (CHP) is a comprehensive medical plan, sometimes called a Health Maintenance Organization or HMO. Unlike traditional insurance, which pays claims after care is furnished, this Plan arranges in advance with physicians and hospitals to provide care to members and directly pays those providers for their services. Because CHP directly provides care and pays the cost, it seeks efficient and effective delivery of health services. By controlling unnecessary or inappropriate care, CHP can afford to offer a more comprensive range of benefits for accidents, illness, and injury. CHP emphasizes preventive services such as office visits, physicals, immunizations, and well-baby care. Covered benefits are available only from CHP physicians and affiliated providers, except during a medical emergency.

Originally established in 1982, Capital health Plan was the area's first health maintenance organization. It was created by local citizens concerned with the rising cost of health care and the need for an alternative system of health care delivery. Since its inception, CHP successfully has insulated its members from many of the problems and concerns being experienced nationally in the health care delivery system. Although costs have gone up, the level of premium increase for CHP has remained below national averages.
Capital Health Plan continues to seek ways to better serve its members and the community by emphasizing our commitment to quality, service, and cost-effective care. Over 2300 area businesses, both large and small, currently provide their employees with access to membership in Capital Health Plan. Step toward health with Capital Health Plan.
- Capital Health Plan's Premier Plus Selection and Premier Selection plans with prescription drug option are available for groups with 51+ employees. Premier Plus Selection has lower copayments and the Premier Selection offers slightly higher copays.
- Capital Health Plan's Essential Selection, Standard Option 1, Standard Option 2, Basic Option 1 and Basic Option 2 plans are available for groups 1 to 50 eligible employees. Prescription drug coverage is included with the Standard and Basic Plan option and is optional with the Essential Selection.
- All One Life Groups have an annual open enrollment period during the month of August with an effective date of October 1.
