Centerville Place Health Center
2140 Centerville Place, Tallahassee, FL 32308 (across from the U.S. Post Office)
The Capital Health Plan Health Center at Centerville Place is home to Capital Health Plan’s Urgent Care Center, open seven days a week by appointment. The Centerville facility also offers physicians’ offices, x-ray and lab services, Capital Health Plan’s Colon Screening Program, and an eye care center. All of these services are available exclusively to Capital Health Plan members!

Governor's Square Health Center
1491 Governor's Square Blvd, Tallahassee, FL 32301 (near the movie theater)
The Governor’s Square health center offers physicians’ offices, Imaging Center, lab services, and an eye care center. This facility is also home to Capital Health Plan’s Center for Chronic Care. All of these services are available exclusively to Capital Health Plan members!
